    Lori will have the information needed to begin your Needs Assessment and make the most of your time together.

What to bring

  • Photo ID
  • Original Medicare ID Card if you already have itThis is your federally issued, red, white, and blue Medicare card showing your Medicare number and Part A and Part B effective dates.
  • Your current medication and provider information if you haven't already provided it.

Comparing Employer Coverage to Medicare?

Please also bring information about your current employer health coverage, including:

  • Your health insurance ID card
  • How much you pay for your healthcare coverage
  • Your deductible
  • Common copays, such as Primary Care, Specialist, and Hospital
  • Your current prescription costs/copays

This allows us to look beyond the monthly premium and make a more meaningful side-by-side comparison of your current employer coverage and Medicare options.
